The most efficient way to register with us at Poplar Primary Care Centre is using our Online Registration Portal. - Paper Registration (PRF1 Form)
Should you choose to register using the paper version of the PRF1 form, please drop into our friendly reception team, email us via info.poplar@nhs.net or post us to us at Poplar Primary Care Centre, 58 – 64 Poplar Road, Kings Heath, Birmingham, B14 7AG
We aim to have you registered within 5 working days but please allow up to 2 weeks for your registration to be completed. You will receive an SMS to confirm you are successfully registered with us a Poplar Primary Care Centre.
Once registered, if you feel you need medical advice, need to see a GP, have a query regarding your medication or need to order a Fit to Work Note (sick note) please submit an AccuRx Triage, simply complete the form. We will hope to respond to your request either by text message or telephone very quickly. Appointments are offered on a Face to Face basis once your triage request has been reviewed by our Triage Doctor.

Temporary Registration
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
